Health service providers spread across Geelong region and Victoria’s south west offer a wide range of palliative care services for clients to access. From care in the comfort and peace of your own home, to inpatient care in hospitals and aged-care facilities, multi-disciplinary teams work closely with patients to manage symptoms and pain and plan their palliative pathway.

It’s all about collaboration, with health professionals committed to keeping their patients well informed so they can make knowledgeable choices about the management of their own illness.

At Barwon South Western Region Palliative Care Consortium, we play a major role in developing and improving palliative and hospice care services across the region.

Six major health agencies form our alliance. These agencies, which include Barwon Health, provide palliative care services in centres including Geelong, Colac, Warrnambool, Portland, Hamilton and on the Bellarine Peninsula.

While the patient is central to the care, the consortium and health service providers recognise the family and carers of a person living with a life-threatening illness need support too. This support comes in many forms including grief and bereavement counselling and respite. It’s practical support and it’s well received.
