Victorian Virtual Specialist Consults

VVSC supports general practitioners, nurses, allied health professionals and other primary care providers by providing timely access to specialist expertise. VVSC aims to support the management of complex patients in the community, strengthen continuity of care, and ease pressure on outpatient departments across Victoria.

2. Visiting Services
Barwon Health In Reach Service

The Residential In reach Program (RIRP) can provide assessment and short term intervention when a resident is acutely unwell and at risk of presentation to ED, when the regular GP is unavailable. RIRP also provides follow up for complex hospital discharges, or those at risk of re presentation.

Barwon Health Community Palliative Care

Community Palliative Care provides specialist care in a client’s home or outpatient clinic as appropriate. Our aim is to ensure that the most appropriate mix of services and professionals are available to provide care and support to patients and their family/carers. Community palliative care specialist staff provide:
Assessment for symptom management
Advice and support utilising a needs-based model of care
