Barwon Health In Reach Service
The Residential In reach Program (RIRP) can provide assessment and short term intervention when a resident is acutely unwell and at risk of presentation to ED, when the regular GP is unavailable. RIRP also provides follow up for complex hospital discharges, or those at risk of re presentation.

Community Palliative Care provides specialist care in a client’s home or outpatient clinic as appropriate. Our aim is to ensure that the most appropriate mix of services and professionals are available to provide care and support to patients and their family/carers.
