Become an Advance Care Planning Facilitator
This face to face workshop is designed to enhance advance care planning knowledge,
focusing on “how to have the conversation” in a practical and interactive way.
Training: Part 1—online E-Learning
Pre requisite E– Learning Campaigns 1, 2 & 3 are compulsory and must be completed prior to
attending the ACP Workshop Day and certificates produced for verification purposes.
Part 2—Full Day Workshop
Learning Outcomes:
• Identify opportunities or triggers in routine practice when advance care planning discussions
are indicated.
• Feel comfortable and confident to initiate an advance care planning discussion with patients and their
families.
• Learn and practise the skills required to hold an advance care planning discussion.
• Describe some of the legal and ethical implications of advance care planning in the healthcare setting.
• Be able to support people to complete Advance Care Directives.
